  • Advancing Integrated Behavioral Health for Federal Grantees

CCBHC Grant Support

The Challenge

 

Certified Community Behavioral Health Clinics (CCBHCs) are at the forefront of transforming how behavioral health services are delivered—providing integrated, person- and family-centered care for individuals with complex mental health and substance use needs. Backed by the Substance Abuse and Mental Health Services Administration (SAMHSA), these clinics are designed to expand access, improve care coordination, and deliver measurable outcomes for vulnerable populations.

Across multiple states and regions, CCBHC grantees face the challenge of implementing this comprehensive care model while simultaneously building the infrastructure to measure, report, and sustain impact. Clinics need to integrate behavioral health and primary care services, expand crisis response capabilities, and demonstrate improvements in outcomes such as quality of life, substance use reduction, and cost of care.

At the same time, grantees are required to meet rigorous federal reporting standards, align stakeholders, and develop long-term sustainability strategies—all while serving diverse populations, including individuals with serious mental illness, substance use disorders, and co-occurring conditions.

OUR APPROACH

A-G designed and executed a cross-site evaluation examining healthcare utilization and cost outcomes across multiple Maryland CCBHCs. This effort included stakeholder engagement, environmental scans, and data-driven recommendations—equipping policymakers with evidence to inform decisions around certification and funding.

We developed tailored evaluation frameworks, logic models, and performance metrics to track patient outcomes and program effectiveness.

Team A-G produced 30+ reports for SAMHSA and translated complex data into actionable insights for leadership and policymakers

Our efforts strengthened data collection, reporting processes, and organizational capabilities through training and technical assistance.

Enhanced communication across federal, state, and community stakeholders, including policy briefs led to better, more informed decision-making.

THE Impact

 

A-G's support of a growing portfolio of CCBHC's has enabled grantees to more effectively track and demonstrate patient outcomes while strengthening the integration of behavioral health and primary care services. Clinics have expanded access to crisis response and coordinated care for thousands of individuals, while building stronger internal capabilities for data-driven decision-making and long-term sustainability. At the same time, A-G’s evaluation and reporting efforts helped inform policy discussions, contributing to the continued growth and support of the CCBHC model.

Turning data into action, Team A-G is strengthening care delivery, improving outcomes, and positioning clinics for long-term success.
